In the context of the development and profitability of its operations, and even the distribution of its products, Chanas Assurances S.A.<\/strong> authorizes itself to establish business relationships with qualified individuals and legal entities in accordance with the provisions of Articles 501 and 533 of the CIMA Code<\/em>.<\/p>\n

To this end, the constitution of the documents to be produced is as follows, according to the nature of the Intermediaries required:<\/p>\n<\/div>

  1. BIRTH CERTIFICATE<\/strong> or SUPPLEMENTAL JUDGMENT<\/strong> in lieu thereof, dated less than three (03) months<\/em> ago;<\/li>\n
  2. EXTRACT FROM THE JUDICIAL RECORD<\/strong> dating less than three (03) months<\/em>;<\/li>\n
  3. PROFESSIONAL<\/strong> DIPLOMAS<\/strong>;<\/li>\n
  4. CIMA DECLARATION FORM<\/strong>, approved by the Public Prosecutor<\/strong> of the Court of First Instance ;<\/li>\n
  5. CERTIFICATE OF NATIONALITY<\/strong>;<\/li>\n
  6. Two (02) 4 x 4 PHOTOS<\/strong>;<\/li>\n
  7. BUSINESS PLAN<\/strong>;<\/li>\n
  8. CERTIFIED COPY<\/strong> of the original NATIONAL IDENTITY CARD<\/strong> (CNI) ;<\/li>\n
  9. A CURRICULUM VITAE<\/strong>;<\/li>\n
  10. Unique Identification Number<\/strong> (UIN);<\/li>\n
  11. Any other document deemed necessary.<\/li>\n<\/ol>\n<\/div>
